To register by mail; please download, print, fill out, and return the form with payment to the address provided. Please note that for Early Registration we must receive your form with payment by April 15th.  For Regular Registration we must receive your mail in form with payment by May 15th.

The CE Seminar will be broadcast live from 9:00-5:30 (Central Time) on May 20th, 2021.
